PDO Threads: The Non-Surgical Facelift Explained

For patients who want to address sagging skin and loss of facial definition but are not ready for surgical intervention, PDO thread lifts offer a compelling middle ground. This minimally invasive procedure uses dissolvable sutures to physically lift and reposition tissue while simultaneously stimulating your body's collagen production for long-term improvement. At Lodi MedSpa, PDO threads have become one of our most transformative non-surgical treatments.

What Are PDO Threads?

PDO stands for polydioxanone, a biocompatible synthetic material that has been used in surgical sutures for over 30 years. In aesthetic medicine, PDO threads are sterile, absorbable threads inserted beneath the skin using thin needles or cannulas. Once placed, they provide an immediate mechanical lift to sagging tissue. Over time, they stimulate your body's fibroblast activity, triggering new collagen and elastin production along the thread pathway. The threads themselves dissolve naturally within 6-8 months, but the structural collagen they stimulate persists for 12-18 months or longer.

Types of PDO Threads

There are two main categories of PDO threads, and most treatment plans use a combination of both. Smooth threads (also called mono threads) are thin, smooth threads placed in a criss-cross or mesh pattern beneath the skin. They do not physically lift tissue but instead create a scaffolding effect that stimulates collagen production, improving skin quality, thickness, and mild tightness. Smooth threads are excellent for the neck, under-eye area, and areas where you want texture and tightening improvement without significant lifting.

Barbed threads (also called lifting threads or cog threads) have tiny barbs or hooks along their length that grip tissue and physically lift it into a more youthful position. These are the threads that provide the dramatic lifting effect most people associate with thread lifts. Barbed threads are used for jowls, mid-face sagging, nasolabial folds, marionette lines, brow lifting, and jawline definition.

The Procedure

A PDO thread lift at Lodi MedSpa typically takes 45-90 minutes depending on the number of areas treated. The procedure begins with cleansing the treatment area and applying local anesthetic for comfort. Your provider will mark the insertion points and thread pathways based on your facial anatomy and desired outcome. The threads are then inserted using thin needles or cannulas, positioned precisely to achieve the optimal lift. Once placed, the skin is gently smoothed and the threads are secured. You will see an immediate lifting effect, though final results become apparent once post-procedure swelling resolves.

Recovery and What to Expect

Recovery from a PDO thread lift is significantly faster than surgical facelifting. Most patients experience 3-7 days of mild swelling, tenderness, and possible bruising. During the first two weeks, you should avoid opening your mouth very wide (no biting into large sandwiches or apples), sleep on your back with your head elevated, avoid strenuous exercise and heavy lifting, skip facial massages and aggressive skincare, and refrain from dental work if possible. Most patients return to work and social activities within 3-5 days. Swelling resolves gradually over 2-4 weeks, revealing your final lifted result.

Results and Longevity

PDO thread results unfold in two phases. The immediate phase provides a visible lift right after the procedure, though some of this is partially obscured by initial swelling. The regenerative phase occurs over the following 3-6 months as your body produces new collagen along the thread pathways, further improving skin quality and tightness. Total results typically last 12-18 months. Many patients choose to have maintenance treatments annually to sustain their results. PDO threads can also be combined with neurotoxins, fillers, and skin resurfacing treatments as part of a comprehensive non-surgical rejuvenation plan.

Is a Thread Lift Right for You?

The best candidates for PDO threads are patients with mild to moderate skin laxity who want meaningful improvement without the cost, downtime, and risks of surgery. If you are noticing early jowling, loss of jawline definition, sagging mid-face, or a heavy brow, a thread lift may be an excellent option. During your consultation at Lodi MedSpa, your provider will assess your skin laxity, facial anatomy, and goals to determine if PDO threads are the right approach — or if a different combination of treatments might serve you better.
